The network pharmacy team includes a senior, prescribing pharmacist and a senior pharmacy technician.
As well as conducting formal structured medication reviews in complex patients the team support patients with new medications and on discharge from hospital.
The pharmacy team are also are invaluable source of knowledge about medications for the rest of the team.
Patients visit their GPs for a range of different reasons and sometimes these issues can be caused by non-medical matters such as loneliness, anxiety or debt. A GP may be able to offer advice or medication to manage the physical or mental impact of these issues but they often cannot help patients to deal with the underlying problem.
Social Prescribing links patients with appropriate, non-medical individual support and advice. In the same way that a patient with a prescription can get their medication easily from a pharmacist, they will be able to quickly access the services that will help them.
Many young people are struggling with how they feel. They are having problems with friends, family or school, they are anxious, depressed, angry or scared and need to talk to someone, but find it difficult to talk to people they know. Counselling and other advice services can help them talk things through, support them without making them feel judged, and in confidence (unless they disclose that their personal safety is at risk). Counselling services can make a positive difference to their lives.
