While GPs and practice nurses remain central, modern primary care is built on a multi-disciplinary network of experts. Depending on your needs, your care may be provided by a dedicated specialist from our PCN team.
PCN Pharmacy Services
Our Pharmacy team consists of Clinical Pharmacists and Pharmacy Technicians.
They are experts in medicines and work alongside your GP to make sure your treatment is safe and effective
Social Prescribing and Care Coordination
Across Crane Valley PCN, our practices provide dedicated non-clinical professionals—including Social Prescribing Link Workers and Care Coordinators—to support you.

About Crane Valley Primary Care Network and what we do
Primary Care Networks (PCNs) bring local GP practices together to deliver proactive, personalised, and coordinated health and social care across our community.
Crane Valley PCN is a collaborative partnership of four local practices: The Barcellos Family Practice, The Cranborne Practice, Verwood Surgery and West Moors Village Surgery, working closely with each other and local community partners to provide the right care, in the right place, at the right time.
